Meridian Services offers person-centered supports and cares to individuals with mental illness, intellectual disabilities, and physical disabilities across a diverse range of settings. These include Residential Services, In-Home Services, and Case Management Services.

Program Manager - In-Home Services
Location: Long Prairie, MN and surrounding areas Schedule: Monday - Friday, typical daytime business hours with availability for evenings and weekends based on needs of the program and staffing. Managers are on-call for their program during the week and rotate being on-call during the weekends and holidays with a team of managers.
Wage: $47,000 annual salary including Full Time benefits
JOB SUMMARY
The In-Home Services Program Manager will manage a caseload of 30-35 individuals with intellectual disabilities and/or mental health diagnoses in the Twin Cities and surrounding areas. Provide 11 support to individuals in their homes and communities. Responsibilities include:
• Supervising and training in-home staff • Overseeing care programs • Conducting meetings • Ensuring compliance with licensing regulations • Providing direct care • Leading meetings with parents/guardians, teachers, and/or work programs and case managers • Assisting with the development and implementation of person-centered plans and outcomes • Writing progress reviews and making recommendations to the team on changes • Assisting in writing behavioral programming with the Administrator as applicable per person served • Completion of person served paperwork in compliance with 245D • Developing and connecting with community resources to ensure successful community integration • Financial reports based on projections and actuals for billable hours on a monthly basis • Completion of staff and person served calendars • Payroll and billings bi-weekly • Documentation of contact and ensuring excellent customer service to persons served, staff, and team members
Preferred Qualifications
Experience supervising staff in a social service field Certificate or bachelors degree in human services or related area Experience working direct care with individuals with autism Experience working with individuals with verbal and physical aggressions Comfortable with personal cares Experience working with Emergency Use of Manual Restraints Incident
Required Qualifications
Must be at least 21 years of age Have a vehicle with current car insurance Have an acceptable driving record as determined by Meridian Services policy Successful clearance from a DHS Background Study
