BRIEF DESCRIPTION:

The purpose of this grant is to increase local dementia support, support persons with Alzheimer's Disease and Related Dementias (ADRD) and their Caregivers, and promote Early Detection and Diagnosis of ADRD. This position reports to the Wellness Programs Administrator and supervises one part-time Health Educator. This position will secure dementia resource specialist training for staff; conduct a community needs assessment or analyze recent assessments to identify community needs; strengthen existing or secure new referral relationships with local healthcare providers; gather and disseminate resources to persons with ADRD and the community; provide education to persons with ADRD and their caregivers on topics identified in the community needs assessment; and act as a referral entity for persons with ADRD, their caregivers, and healthcare providers to resources.

This position is responsible for programmatic and financial oversight of the ADRD grant, including ensuring compliance with grant guidelines, monitoring program expenditures, and compiling data necessary for programmatic and fiscal needs.

LICENSING REQUIREMENTS:

Driver's License Required - Y Type - C

OTHER REQUIREMENTS:

Experience in community health education, community engagement, and supervision are desired. Ability to work evenings, weekends, and extended hours.

2 L Achieves grant goals by submitting grant objectives and activities, developing and maintaining activity reports, ensuring all deadlines are met, coordinating activities with both internal and external entities, and working within established standards and regulations.

3 L Supervises staff by scheduling and assigning tasks, recommending additional activities, ensuring tasks are within guidelines, providing training and education, completing evaluations, and overseeing all outreach activities.

4 L Assists with identifying community needs related to ADRD and gaps in existing resources.

5 L Monitors budget and provides reports, as required. Ensures compliance with fiscal monitoring standards for grants.

6 L Develops and implements a plan to share resource information with the community.

7 L Disseminate resources to persons with ADRD, their caregivers, and providers in all identified communities.

8 L Strengthens and develops referral relationships with local healthcare providers and community organizations.

Experience-Related Requirements Over one year and up to and including two years.

Vocational/ Educational Preparation Requires Bachelor's degree or education and training equivalent to four years of college education in business, liberal arts, or any field other than engineering or the hard sciences.
