Job Summary:
Provides primary clinical management and coordination of care for Cystic Fibrosis patients. Will provide coordination of care across the continuum for this pulmonary population. The coordinator will work with patients and families directly, along with the team of providers, social workers, support staff, inpatient colleagues, 3rd party payors and community resources. The coordinator will be able to provide services according to the Nursing Scope of Practice as well as coordinate quality improvement initiatives for this patient population. Other duties as assigned.
EXPERIENCE DESCRIPTION:
Three to five years of experience in nursing field is required. Patient care experience in an acute care, intensive care or Ambulatory Clinic setting is required.
EDUCATION DESCRIPTION:
Bachelor's degree in Nursing is required
SPECIAL SKILLS DESCRIPTION:
High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative, problem solve, and be proactive in assisting the team in managing the population.
LICENSURE DESCRIPTION:
Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days.
