The Wisconsin Medical Home Initiative is a key initiative of Children's Health Alliance of Wisconsin (Alliance) affiliated with Children's Wisconsin. The Alliance is a statewide child health advocacy organization whose mission is to ensure children are healthy, safe and able to thrive. This mission is focused on key health initiatives and involves working closely with diverse partners, including multiple state agencies, health care systems, and statewide organizations to impact public policy, access to care and quality health outcomes. The Medical Home program manager specifically works on Maternal and Child Health goals with local public health departments, health care professionals and community leaders to support early identification and coordination efforts at the state and local level.

Activities will include bringing diverse sectors together to improve integrated systems of health by leveraging the following: outreach and education, policy and practice, and data analysis and evaluation. Responsibilities will include managing work plans, building strategic partnerships, provide trainings and technical assistance on the topics of developmental surveillance and developmental screening tools,

and implementing evidence-based strategies to ensure that kids and families experience coordination of services.

Job Summary

Manages a specific program within an initiative at Children's Health Alliance of Wisconsin (Alliance) affiliated with Children's Wisconsin. Primary duties include managing the program work plan, building partnerships and implementing strategies. Facilitate group or coalition meetings, create and deliver training, and/or program content.

Essential Functions

  • Identifies, creates and executes processes necessary to meet specific project compliance, standards and requirements, including managing timetables, work plans and budgets.

  • Contributes to the creation of strategies and manages the implementation of strategies identified in the work plans and/or grant documents.

  • Provides input and strategic direction on public policy, partnership development and implementation of strategies, including the mobilization of stakeholders.

  • Facilitates meetings among partners and prepares informative and well-organized presentations as needed.

  • Collaborates with state, local and national partners to identify and carry out project strategies.

  • Engages in continuous quality improvement methodology in implementing strategies that advance child health and the AIM of the initiative.

  • Provides content for social media (Facebook, Twitter, and Blogs) and the Alliance website.

  • Provides regular updates to the Alliance executive director and leaders as appropriate.

  • Serves as a liaison between state and national grant funders, program staff, and other organizations working to advance child health in collaboration with program leaders or executive director.

  • Represents Children's Wisconsin and the Alliance throughout Wisconsin and nationally in a professional manner, including serving on local, regional, and statewide committees, coalitions, advisory committees, boards and other groups at the direction of their leader or director.

Education:

  • Bachelor's Degree in community health, public health, human services, social work, business management or other health related field required

  • Master's Degree related field preferred

Experience:

  • 2+ years experience managing a project or related experience required

Knowledge, Skills and Abilities

  • Interest and ability in working collaboratively in a team environment and the ability to work with diverse groups and individuals.

  • Proven ability to initiate and manage multiple projects within set timeframes.

  • Skills, knowledge, formal experience with training others

  • Effective public speaking experience and skill.

  • Effective and professional oral and written communication skills including the ability to develop reports and presentations.

  • Knowledge and experience in the grant writing process and/or direct experience in writing and preparing grants is preferred.

  • Excellent computer skills and well-versed in the Windows environment.

  • Acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.

Licenses and Certifications:

  • License Driver's License - State of Wisconsin required
