Job Title: PRN (Per Diem) Home Health Aide (HHA) – Field/Travel
Location: Pennsylvania (various patient homes within assigned service area)
Position Summary:
The PRN Home Health Aide provides compassionate, hands-on personal care to patients in their homes on an as-needed basis. This role requires travel between patient locations and flexibility in scheduling. The aide supports patients with daily living activities while promoting comfort, dignity, and independence.
Key Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting
- Provide mobility support, including transfers, ambulation, and positioning
- Perform light housekeeping (laundry, dishes, tidying patient areas)
- Prepare simple meals and assist with feeding when needed
- Monitor and report changes in patient condition to supervising nurse or agency staff
- Maintain accurate documentation of care provided
- Follow individualized care plans and safety protocols
- Travel to multiple patient homes within assigned geographic area
Schedule:
- PRN / Per Diem (as needed) required four shifts per month
- Flexible shifts based on patient demand
- May include days, evenings, weekends, and holidays
Qualifications:
- High school diploma or GED preferred
- Completion of a state-approved Home Health Aide training program (if required)
- CNA certification (preferred but not always required)
- Current CPR certification (preferred)
- Valid driver’s license, vehicle, auto insurance, and valid registration
- Ability to pass background check and drug screening

- Physical ability to assist with lifting, standing, and mobility support
Compensation & Benefits
- Hourly pay (typically $16–$18/hour depending on experience and assignment)
- Mileage reimbursement (if applicable)
- Flexible scheduling
Work Environment
- Patient homes in community settings
- Frequent travel required
- Exposure to various home environments and conditions
- Physical Requirements
- Ability to lift, push, or pull up to 25–50 lbs
- Prolonged standing, walking, bending, and assisting patients
