Palliative Care LPN – PRN | Per-Visit Pay | Cleveland / Northeast Ohio

VNA of Ohio is one of Northeast Ohio's most established home health agencies - 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're building our PRN Palliative Care LPN team and offering competitive per-visit pay with a flexible schedule.


Pay Rates – Per Completed Visit

Per-Visit - $42

Downtime (pre-approved wait time) - $30/hr

"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.


Service Area & Schedule

Cleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand - with a flexible schedule.


What You'll Do

  • Provide direct patient care to palliative care patients in their homes in accordance with the established plan of care
  • Monitor and document patient symptoms, comfort levels, and changes in condition
  • Administer medications and treatments as directed by the RN Case Manager or physician
  • Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
  • Provide education and emotional support to patients and families managing serious illness
  • Maintain accurate and timely documentation in the EMR within required timeframes

Requirements

Must-Have:

  • Active LPN license (Ohio) + current BLS certification
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Minimum 1 year of clinical nursing experience

Preferred / Nice-to-Have:

  • Prior palliative care, hospice, or home health experience
  • Experience supporting patients and families through serious illness
  • Familiarity with HCHB or similar home health EMR
  • Strong communication and family support skills
