Givens Estates is looking for a PRN RN Navigator to support our Resident Services team. This position is critical in promoting the health and well-being of the residents through proactive education, effective facilitation of health programs, and guiding residents to appropriate resources. The Nurse Navigator will help create a supportive and informed community, fostering a better quality of life for all residents.

What youll do:

  • Serve as a resource for residents seeking information on healthcare services, insurance, and community resources
  • Provide referrals to specialized healthcare professionals when necessary
  • Coordinate and facilitate support groups for residents dealing with specific health issues like diabetes, heart disease, and mental health challenges
  • Provide one-on-one educational consultations to residents and their families about health-related matters
  • Maintain an up-to-date directory of local health care providers, support services, and other relevant resources
  • Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What youll need:

  • Bachelors or Masters prepared nurse
  • Minimum 2 years of care coordination/case management experience
  • Strong organizational skills
  • Experience with geriatric population

Compensation is based on background and experience
