Location: Woodland Heights Medical Center, Lufkin, TX

Schedule: PRN- day shift- Flexible hours

Job Summary

The Social Worker - LMSW provides professional social work services to patients and families to address psychosocial needs, support care transitions, and connect individuals to community resources. This role performs comprehensive assessments, provides emotional support and counseling, and collaborates with the interdisciplinary care team to develop and implement effective treatment and discharge plans. The Social Worker - LMSW plays a key role in improving patient outcomes, reducing readmissions, and supporting vulnerable populations across the care continuum.

Essential Functions

  • Conducts age-appropriate psychosocial assessments for patients across the lifespan, including neonatal, pediatric, adolescent, adult, and geriatric populations.
  • Assesses the interrelationship between psychosocial factors and medical conditions to identify barriers to recovery and discharge.
  • Provides emotional support, crisis intervention, and counseling to patients and families coping with illness, injury, grief, disability, or hospitalization.
  • Identifies social, emotional, and financial needs and facilitates referrals to internal and external support services.
  • Develops, documents, and follows through on individualized treatment and discharge plans in coordination with the interdisciplinary team.
  • Manages complex, high-risk referrals involving suspected abuse, adoption, fetal demise, end-of-life care, behavioral health, and psychosocial crises.
  • Supports the transition of care by coordinating services such as home health, hospice, rehabilitation, and long-term care placement.
  • Facilitates or participates in support groups and patient/family education sessions when appropriate.
  • Maintains timely, accurate documentation in accordance with regulatory and organizational standards.
  • Participates in interdisciplinary rounds and collaborates with physicians, nurses, case managers, and other healthcare professionals to ensure safe and effective care planning.
  • Performs other duties as assigned.
  • Maintains regular and reliable attendance.
  • Complies with all policies and standards.

Qualifications

  • Master's Degree in Social Work (MSW) or Psychology required
  • 2-4 years of experience in a healthcare or clinical social work setting preferred

Knowledge, Skills and Abilities

  • Knowledge of psychosocial assessment techniques, clinical social work principles, and healthcare discharge planning.
  • Strong understanding of community resources, social service systems, and care coordination best practices.
  • Excellent communication, counseling, and conflict resolution skills.
  • Ability to manage emotionally complex situations with professionalism and empathy.
  • Strong organizational and documentation skills with attention to regulatory requirements.
  • Ability to collaborate with diverse teams and work in a fast-paced healthcare environment.
  • Familiarity with electronic medical records and case documentation systems.

Licenses and Certifications

  • LMSW - Licensed Medical Social Worker - State Licensure required
  • BCLS - Basic Life Support obtained within 7 days of employment required
