What Youll Do

Provide skilled nursing care in accordance with physician orders and care plans

Review and follow the patient Plan of Care before each visit

Monitor patient conditions and communicate changes with the RN Case Manager and care team

Educate patients and caregivers on treatment plans and self-care techniques

Support patient safety initiatives and identify opportunities to prevent avoidable hospitalizations

Administer treatments and document care accurately and timely

Collaborate with physicians and interdisciplinary team members to ensure quality patient care

Deliver compassionate care and exceptional service to patients and families

What Youll Need

Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

Current Indiana LPN license

Minimum of 1 year of nursing experience in a clinical care setting

Home Health or Hospice experience preferred

Ability and willingness to travel within assigned territory

Valid driver's license, current auto insurance, and reliable transportation

Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
