Compensation:$26-$28/ Houre DOE

Status: Full-Time, W2 Employee

Schedule: Monday - Friday, 8 AM - 5 PM

About the Role

We are seeking a compassionate and dynamic Primary Care Supervisor to join our Atlas Medical Team. A Primary Care Supervisoris responsible formaintaining and growing relationships with existing referral sources and customers such as hospitals, physician offices, case managers, discharge planners, long-term care facilities, and home health agencies. The focus is on the accountretention, service quality, and revenue growth within assigned territories.

Your key responsibilities will include:

Team Supervision & Development

  • Supervise all Atlas departments and employees.
  • Conduct regular one-on-one meetings with team members.
  • Review outcomes with the Atlas Practice Manager.
  • Hold team members accountable for workflows and deadlines.

 

Operational Coverage & Training

  • Learn all department roles and workflows.
  • Provide coverage during employee absences.
  • Identify process gaps and support improvements.

 

CCM Program Oversight

  • Manage the Chronic Care Management (CCM) program.
  • Create and maintain care plans.
  • Meet regularly with Sue regarding CCM programs.

 

Communication & Monitoring

  • Monitor facility team threads and ensure timely responses.
  • Collaborate closely with the Atlas Practice Manager.
  • Handle calls from upset patients or facilities professionally.

Qualifications

To thrive in the role of Primary Care Supervisor you must:

  • Requires 2–5+ years in management / supervisor role.
  • Strong leadership and communication skills.
  • High attention to detail and accountability.
  • Ability to multitask and learn multiple workflows.
  • Possess a valid driver's license, clean driving record, and reliable automobile.
  • Provide proof of current automobile insurance (MVR required).
  • Have a clear negative TB test.
