Position Overview
This is a clinic-based primary care role focused on delivering comprehensive, evidence-based care. You'll work alongside a strong interdisciplinary team to support prevention, chronic disease management, and overall patient wellness.
Responsibilities
- Provide direct patient care in a clinic setting
- Perform Annual Health Assessments (AHA) and Chronic Care Management (CCM)
- Focus on health promotion, disease prevention, and patient education
- Collaborate with interdisciplinary teams to coordinate care
- Utilize EHR systems to manage patient care and documentation
- Board Certified or Board Eligible Physician
- Active CA medical license (or ability to obtain)
- Completed residency in Family Medicine, Internal Medicine, or related field
- 1–3+ years of clinical experience preferred
- Strong communication and teamwork skills
- Experience with EHR systems (Epic preferred)
- $230,000 – $300,000 base salary
