Primary Care Physician for Sickle Cell Medical Home

Memorial Healthcare System in South Florida is seeking a board-certified or board-eligible Family Medicine or Internal Medicine physician with a strong interest in the care and management of patients with sickle cell disease. This full-time, employed opportunity with the multi-specialty Memorial Physician Group, and offers the opportunity to work in collaboration with a board-certified hematologist who serves as the Medical Director of a dedicated sickle cell medical home.

Position Highlights:

• Primary focus on the clinical management of adults with sickle cell disease

• Expertise in outpatient pain management of sickle cell disease

• Opportunity to join a comprehensive, multidisciplinary care team that includes a social worker, pharmacist, wellness coach, and dedicated nurses

• Access to dedicated outpatient hematology clinics, infusion/day hospital services, and embedded primary care

• Access to apheresis and transfusion services

• Collaborate with a board-certified hematologist medical director and a team of advanced practice providers dedicated solely to the care of adult patients with sickle cell disease

.• Strong collaboration with Memorial's pediatric hematology team

• Collaboration with Memorial Cancer Institute who partners with Moffitt Cancer Center

• The program is an active member of the American Society of Hematology Sickle Cell Workshops and the National Alliance of Sickle Cell Centers
