Job Overview
We are seeking a compassionate and patient-centered Physician to provide high-quality outpatient medical care to individuals and families across multiple clinic locations in San Jose. This role focuses on delivering comprehensive primary and preventive care while collaborating closely with multidisciplinary care teams to support the overall health and wellness of the community.
The ideal candidate is clinically strong, collaborative, and passionate about serving culturally diverse populations. This physician will evaluate, diagnose, and treat patients while emphasizing preventive care, chronic disease management, patient education, and whole-person wellness.
Key responsibilities include performing physical examinations, reviewing medical histories, ordering and interpreting diagnostic tests, developing treatment plans, prescribing medications, and coordinating referrals to specialists and inpatient services when needed. The Physician will also educate patients on disease prevention, self-management, and healthy lifestyle practices while working closely with behavioral health and wellness teams to support integrated care delivery.
Additional responsibilities include maintaining accurate electronic medical records, participating in clinical quality improvement initiatives, peer reviews, clinical audits, health outcomes reporting, and program evaluations. This role may also provide supervision and mentorship to mid-level providers, residents, and medical students and participate in an after-hours on-call rotation as part of the care team.
