**Job Summary:**
The Board Certified Family Practice Nurse Practitioner / Physician Assistant will provide primary health care services to patients under the general supervision of the Chief Medical Officer at the Community Health Center. The incumbent will play a key role in delivering high-quality care to the patient population by working collaboratively with the medical staff.

**Qualifications:**
- Board certification as a Family Practice Nurse Practitioner or Physician Assistant.
- Current license to practice as a Nurse Practitioner or Physician Assistant in the state.
- Strong knowledge and experience in providing primary health care services.
- Excellent communication and interpersonal skills to interact effectively with patients and medical staff.
- Ability to work both independently and as part of a team in a healthcare setting.
- Commitment to maintaining professionalism, punctuality, regular attendance, cooperation, organization, and enthusiasm in all duties.

**Responsibilities:**
