Practice facilitation, also referred to as quality improvement peer coach, is an approach to supporting improvement in primary care practices that focuses on building organizational capacity for continuous improvement. As a practice facilitator, you will establish a long-term relationship with your practices, becoming a resource for ongoing quality improvement (QI) and evidence translation. PCPF's are specially trained individuals who work with primary care practices to make meaningful changes designed to improve patients outcomes. Is accountable for facilitating the implementation of practice transformation including baseline assessment of current environment, and ongoing facilitation of practice improvement methodologies to ensure compliance with regulatory and NCQA standards. Dispute Resolution For day-to-day operations, the Patient Centered Transformation Facilitator (PCPF) will report to the Patient Centered Transformation Practice Lead who reports to the Manager of Outcomes. The PF will support the development and maintenance of the Patient Centered Medical Home accreditation, and when applicable the Patient Centered Specialty Practices. The PF will support all members of the care team and provide educational support for all quality initiatives tied to the practices they support. Practice locations will vary based on an ever-changing mix of practice locations. Completes other special assignments and projects as assigned, including but not limited to work on Commercial and Medicare quality contracts, Roster Management, Health and wellness campaigns.
Responsibilities:A day in the life of an Practice facilitator at Hackensack Meridian Health includes:
Current quality initiatives include but not limited to; Quality Measure Management (Hedis and MIPS), Reporting and Dashboarding for quality measures, action planning with the physician practices, HMHMG Panel Management activities and Annual Wellness activities, Patient Centered Medical Home accreditation, Social Determinants of Health. These quality transformation projects are subject to change. Must display core traits and behaviors consistent with HMHN 5 Culture during the 90-day orientation period: Creative, Compassionate, Collaborative, Courageous, and Connected. Practice Facilitator supports the work of the Ambulatory Quality and Patient Safety (AQPS) team in creating and sustaining an environment that fosters team member engagement. Coaches and supports assigned practice members as defined by the manager. For assigned practices, serves as the primary liaison, teacher, coach, and facilitator for practices participating in practice transformation activities and facilitates improvement toward measurable outcomes with increasing practice independence over time. Works with practices to understand quality results, and develop plans to improve Assures auditing of practices to maintain compliance with quality initiatives CMS and NCQA standards. Maintain databases directly associated with operational quality role. Acts as a liaison between the Quality leadership and the Ambulatory practices. Facilities Quality Improvement tactics, mentoring physician practice team members and implementing action planning based on outcome measures. Participate in the planning, development, presentation, and delivery of educational programs in support of CMS, NCQA PCMH and value based commercial payer standards. Review patient satisfaction results with practice and develop action plans as needed.
