ROLE AND PURPOSE

The major responsibility of the Post Secondary Facilitator is to provide teachers, students, and parents resources in the development of Transition Plans as determined by State Performance Plan Indicator 13.

QUALIFICATIONS

Education Certification Bachelors Degree with teaching certificate in special education Experience Three years teaching experience in high school special education preferred

MAJOR RESPONSIBILITIES AND DUTIES

I. INSTRUCTIONAL MANAGEMENT

  • Arrange guest speakers regarding various supports as it related to post-secondary goals.

II. STUDENT MANAGEMENT

  • Conduct presentations and training on the transition process including active and interactive website information.
  • Collaborate with the students teacher in completing applications and making post-secondary contacts.
  • Complete the summary of performance.
  • Follow-up on all selected graduated special education students for State Performance Plan Indicator 14.

III. SCHOOL COMMUNITY RELATIONS

  • Participate in after-school parent night informational sessions as it related to transition planning.
  • Meet individually as requested with students and their parents to discuss transition needs.

IV. PROFESSIONAL GROWTH AND DEVELOPMENT

  • Provide support to special education teachers and support staff through training and modeling in the student information gathering process for the completion of transition assessments and transition IEPs.

V. MISCELLANEOUS

  • Make available to students, parents, and teachers the utilization of the AISD and TEA Transition Resource Guide and other available resources that facilitate movement toward the post-secondary goals.
  • All other duties assigned by the Director of Special Education.
