Practice Highlights/Qualifications:
Provide primary care to patients in a post-acute skilled nursing and rehabilitation setting
Manage complex medical patients transitioning from hospital to rehabilitation
Round on patients daily with collaborative interdisciplinary care teams
Work alongside nurse practitioners, therapists, social workers, and nursing staff
Monday-Friday day schedule with shared call responsibilities
Board Certified or Board Eligible in Family Medicine or Internal Medicine required
MD/DO from accredited medical school required
Completion of ACGME-accredited Family Medicine or Internal Medicine residency required
Active or eligible for Arizona medical license
New graduates and experienced physicians welcome
