Overview

Larimer County Department of Health and Environment is seeking a Public Health Nurse to join a vital program serving pregnant and postpartum individuals and their infants.

In this role, you will guide families through pregnancy and early childhood, with a special focus on improving health outcomes during pregnancy and the postpartum period, supporting secure parent-child attachment, promoting early childhood well-being, and addressing the unique needs of families impacted by Opioid Use Disorder (OUD) or co-occurring Substance Use Disorders (SUD). Responsibilities

What youll be doing:

  • Establish and maintain trusting, therapeutic relationships with pregnant and postpartum clients and conduct home visits following nursing practice standards.

  • Conduct comprehensive assessments that address physical health, behavioral health, environmental stressors, social determinants of health, and family strengths.

  • Provide education on pregnancy, infant health, parenting, child development, and safety.

  • Support families navigating OUD/SUD, including relapse prevention strategies, harm reduction education, MAT (Medication-Assisted Treatment) coordination, and overdose prevention.

  • Stay current with perinatal nursing practices, early childhood development, motivational interviewing, trauma-informed care, harm reduction measures and public health principles.

  • Develop and regularly update individualized nursing care plans based on client strengths, goals, and identified needs.

  • Collaborate with multidisciplinary team members, community agencies, and healthcare providers to coordinate services and address barriers.

  • Participate in reflective supervision, case conferences, and ongoing professional development.

Qualifications

All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.

  • Bachelors Degree, or equivalent combination of education and/or experience, in Nursing from an accredited college or university required. Related education or experience may be substituted on a year for year basis.
  • Licensed to practice as a registered nurse (RN) in the State of Colorado required.
  • One (1) year experience in providing health services as a Registered Nurse required.
  • Possession of CPR and First Aid certifications by date of hire required.
  • Possession of a valid Drivers License by date of hire required.
  • One year (1) of experience in specific nursing program relevant to the role they will perform preferred.

Supplemental Information

Schedule

The standard work schedule for this position is Monday through Friday, 8:00 AM to 4:30 PM. During the initial six-month probationary period, the individual hired may telework up to one day per week, if business needs allow. After successfully completing the probationary period, the individual may telework up to two days per week, depending on business plans and operational needs. Additionally, this position includes assignments that require in-community work, which may affect the hybrid remote schedule. Your specific schedule will be discussed with you to ensure it aligns with your roles business needs.

This position is expected to last approximately three years, with the potential for extension. Compensation is typically offered at or below the midpoint of the salary range, depending on experience.

The Department of Health and Environment has a vaccine requirement. Successful candidates will be required to comply with the vaccine requirements or request a waiver.
