Position Summary:
We are seeking a Physician (MD) to deliver on-site rounding and primary care services across a defined territory. You will manage patients in assisted living, SNF, memory care, and long-term care environments, focusing on transitional care, preventative medicine, and complex chronic conditions. This role is ideal for physicians experienced in or interested in home-based primary care and value-based care models.
Responsibilities:
• Provide on-site primary care and rounding services in senior living and SNF settings.
• Manage chronic disease, preventative care, and transitional care needs.
• Reduce hospital readmissions and ER utilization through proactive interventions.
• Perform diagnostics, treatment planning, and medication management.
• Collaborate with NPs, PAs, nursing staff, and interdisciplinary teams.
• Serve as a clinical resource within assisted living and post-acute environments.
• Maintain accurate EMR documentation and coordinate follow-up care.
Qualifications:
• Active Minnesota Physician (MD) license in good standing.
• Experience in geriatrics, internal medicine, family medicine, or SNF/post-acute care preferred.
• Familiarity with SNF rounding, post-acute care, or home-based primary care.
• Strong clinical judgment and ability to manage complex patient populations.
• Valid driver’s license and reliable transportation.
