As a Senior Living Rounding Physician, you'll become a trusted medical leader across the communities you serve. You'll care for complex patients, guide treatment decisions, mentor and collaborate with advanced practice providers, and help shape the overall quality of care delivered throughout your communities.

• Become a clinical leader serving residents throughout senior living communities across Minnesota.
• Build long-term relationships with patients, families, nursing teams, and community leadership.
• Focus on proactive medicine, chronic disease management, preventative care, and reducing avoidable hospitalizations.
• Collaborate with experienced Nurse Practitioners, Physician Assistants, nurses, and interdisciplinary teams.
• Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits.
• Provide on-site rounding and primary care services throughout assisted living, memory care, skilled nursing, and long-term care communities.
• Manage complex medical conditions, chronic disease management, preventative care, and transitional care needs.
• Develop treatment plans and guide clinical decision-making across diverse patient populations.
• Collaborate with advanced practice providers and interdisciplinary teams to ensure continuity of care.
• Support quality initiatives aimed at reducing emergency room utilization and hospital readmissions.
• Communicate effectively with residents, families, caregivers, and community leadership.
• Complete documentation, medication management, and clinical oversight responsibilities.
• Active Minnesota Physician (MD or DO) license in good standing.
• Experience in geriatrics, family medicine, internal medicine, post-acute care, long-term care, or related specialties preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Interest in collaborative, community-based medicine.
• Valid driver's license and reliable transportation.
