You'll care for complex patients, guide treatment decisions, mentor and collaborate with advanced practice providers, and help shape the overall quality of care delivered throughout your communities.

Responsibilities

• Provide on-site rounding and primary care services throughout assisted living, memory care, skilled nursing, and long-term care communities.

• Manage complex medical conditions, chronic disease management, preventative care, and transitional care needs.

• Develop treatment plans and guide clinical decision-making across diverse patient populations.

• Collaborate with advanced practice providers and interdisciplinary teams to ensure continuity of care.

• Support quality initiatives aimed at reducing emergency room utilization and hospital readmissions.

• Communicate effectively with residents, families, caregivers, and community leadership.

• Complete documentation, medication management, and clinical oversight responsibilities.

Qualifications

• Active Minnesota Physician (MD or DO) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, post-acute care, long-term care, or related specialties preferred.

• Strong clinical judgment and confidence managing medically complex patient populations.

• Interest in collaborative, community-based medicine.

• Valid driver's license and reliable transportation.
