We are seeking a dedicated Physician to provide comprehensive medical coverage across Skilled Nursing Facilities (SNF) and Assisted Living Facilities (ALF) in the Long Island market. This role offers the opportunity to lead clinical oversight, manage medically complex patients, and reduce avoidable hospital transfers through proactive, person-centered care.
Coverage Areas:Nassau County, Suffolk County, and select locations in Queens (depending on practice site)
Key ResponsibilitiesSkilled Nursing Facility (SNF) / Post-Acute CareProvide dedicated Medical Directorship with emphasis on quality and compliance
Ensure 7-day medical coverage with on-site and on-call availability
Overnight coverage handled by in-house providers for continuity and responsiveness
Reduce rehospitalization rates below state and national averages
Streamline medication regimens to reduce polypharmacy and pharmacy costs
Ensure appropriate and compliant psychotropic use
Manage medically complex and high-risk patients with expertise and compassion
Implement proactive fall prevention and quality improvement programs
Provide ongoing medical directorship and clinical oversight for residents
Conduct regular on-site visits by physicians and advanced practice providers (APPs)
Maintain 24/7 availability for urgent and after-hours medical needs
Focus on chronic disease management and preventive care
Ensure appropriate psychotropic use and minimize unnecessary medications
Collaborate closely with staff and families for coordinated, person-centered care
Reduce avoidable hospital transfers through proactive intervention
APPs see patients and provide fill-in coverage when needed
Participate in rounds and follow-back care as part of the team
Base Salary:$275,325 + productivity bonus

MD or DO from an accredited medical school
Board Certification (or eligible) in Internal Medicine, Family Medicine, or Geriatric Medicine
Active, unrestricted New York State medical license
Active DEA license
US work authorization
Experience in SNF/ALF settings preferred
Strong commitment to person-centered care for medically complex, high-risk patients
Lead clinical quality and compliance across multiple SNF and ALF sites
Work with a collaborative team of physicians and APPs
Manage complex cases with expertise and compassion
