COMPENSATION & BONUS PACKAGE

$250,000 Base Salary – New Residents
$295,000 Base Salary – Experienced Physicians
Annual Incentive Bonuses: $45K–$85K+
Bonus opportunity up to 28% of annual base salary
Potential Total Compensation: $350K–$380K+ annually

PRIMARY RESPONSIBILITIES

Evaluate and treat patients according to established clinical standards
Coordinate specialty referrals, hospital care, SNF coordination, home health services, and durable medical equipment needs
Participate in daily clinical huddles and collaborative care planning
Maintain accurate and timely EMR documentation
Review quality metrics and patient outcomes with leadership teams
Deliver compassionate, patient-centered primary care
Foster a positive and collaborative team environment

REQUIRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━
✔️ MD or DO from an accredited medical school
✔️ Active and unrestricted medical license (or willingness to obtain)
✔️ Board Certified or Board Eligible in:
• Family Medicine
• Internal Medicine
• Geriatric Medicine
✔️ Active DEA License
✔️ Medicare & Medicaid Provider eligibility
✔️ Strong communication and interpersonal skills
✔️ Comfortable working in a collaborative care setting
✔️ Basic EMR/EHR and computer skills
