As a Physician Assistant focused on wound care and senior living medicine, you'll work with patients who often face complex medical challenges, chronic conditions, and barriers to recovery. Responsibilities

• Provide on-site rounding care throughout assisted living, memory care, and long-term care communities.

• Manage pressure injuries, diabetic ulcers, post-surgical wounds, and skin integrity concerns.

• Support chronic disease management and preventative care initiatives.

• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams.

• Communicate effectively with families, caregivers, and facility staff.

• Complete documentation, medication management, diagnostics, and lab coordination.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in wound care, geriatrics, primary care, internal medicine, family medicine, or post-acute care preferred.

• Strong clinical judgment and communication skills.

• Valid driver's license and reliable transportation.

• Commitment to high-quality, patient-centered senior care.
