As a Physician Assistant with Twin Cities Physicians, you'll have the opportunity to become that provider.
This role combines on-site primary care, advanced wound care, chronic disease management, and geriatric medicine within senior living communities throughout the Twin Cities Metro. You'll care for patients over time, follow their progress, and become a familiar face for residents, families, and caregivers alike.
• $120,000 to $160,000+ total compensation opportunity through salary and productivity incentives.

• Develop expertise in wound care, geriatrics, chronic disease management, and post-acute medicine.

• Provide on-site rounding, primary care, and wound care services.
• Manage pressure injuries, diabetic ulcers, surgical wounds, and skin integrity conditions.
• Support chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
• Communicate effectively with families and interdisciplinary care teams.
• Complete documentation, medication management, diagnostics, and lab coordination.
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in geriatrics, primary care, wound care, family medicine, internal medicine, or post-acute settings preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.
• Commitment to patient-centered senior care.
