Position Summary

As a Physician Assistant with Twin Cities Physicians, you will provide on-site care throughout senior living communities while managing chronic conditions, coordinating care, and helping patients navigate complex healthcare needs. You'll have the opportunity to build continuity with your patient population while practicing in a setting where your clinical judgment truly matters.

Responsibilities

• Provide on-site rounding care throughout assisted living, memory care, and long-term care communities.

• Perform physical examinations, chronic disease management, preventative care, and follow-up visits.

• Develop and adjust individualized care plans based on patient needs.

• Collaborate with physicians, nurses, therapists, caregivers, and community staff.

• Communicate effectively with families and care teams regarding treatment plans and patient progress.

• Support continuity of care and help reduce unnecessary emergency room visits and hospitalizations.

• Complete documentation, medication management, and lab coordination.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, or post-acute care preferred.

• Strong clinical judgment, communication skills, and patient-centered approach.

• Valid driver's license and reliable transportation.
