Position Summary

This is not a traditional Physician Assistant position.

This is an opportunity to become part of a growing specialty area where experienced clinicians can make a significant impact while building highly marketable expertise.

As a Wound Care & Rounding Provider, you'll work with medically complex patients, manage advanced wound cases, and provide ongoing clinical oversight within senior living communities. You'll become a trusted healthcare resource for residents, families, nurses, and community leadership while helping shape outcomes for some of the most vulnerable patient populations in healthcare.

Responsibilities

• Provide on-site rounding and advanced wound care services across senior living communities.

• Manage pressure injuries, diabetic ulcers, surgical wounds, and other complex skin integrity concerns.

• Support chronic disease management and preventative care initiatives.

• Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.

• Help reduce avoidable hospitalizations through proactive clinical intervention.

• Communicate effectively with families, caregivers, and interdisciplinary care teams.

• Complete documentation, medication management, diagnostics, and lab coordination.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in wound care, geriatrics, primary care, post-acute care, internal medicine, or related settings preferred.

• Strong clinical judgment and confidence managing medically complex patient populations.

• Valid driver's license and reliable transportation.

• Commitment to high-quality, outcomes-driven senior care.
