Compensation:$120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

As a Mobile Rounding Provider, you'll serve as a trusted clinical resource within senior living communities, helping patients manage chronic conditions, coordinating care plans, and supporting healthier outcomes. Rather than seeing patients once and sending them elsewhere, you'll become part of an ongoing care model focused on consistency, accessibility, and proactive intervention.

Responsibilities

• Provide on-site rounding care in assisted living, memory care, and long-term care communities.

• Perform assessments, chronic disease management, preventative care, and follow-up visits.

• Develop and adjust patient-centered care plans.

• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams.

• Communicate effectively with families, caregivers, and facility staff.

• Help reduce avoidable hospitalizations through proactive clinical management.

• Complete documentation, medication management, and lab coordination.

Qualifications

• Active Minnesota Physician Assistant (PA-C) license in good standing.

• Experience in geriatrics, family medicine, internal medicine, hospital medicine, home health, or post-acute care preferred.

• Strong clinical judgment and communication skills.

• Valid driver's license and reliable transportation.

• Passion for patient-centered senior care.
