Alliance Medical Clinic and Desert Valley Palliative Care are seeking a compassionate, motivated Physician Assistant (PA) or Nurse Practitioner (NP) to join our growing team in a mobile, community-based clinical role.

This is an exciting opportunity for a provider who enjoys caring for patients outside of the traditional office setting and wants to make a meaningful difference in the lives of medically complex adults and seniors. The role focuses on delivering high-quality mobile primary care, palliative care, and pain management services directly to patients in the settings where they live and feel most comfortable.

Position Overview:
The PA or NP will provide medical care to patients in community-based settings, including:
  • Assisted living facilities.
  • Memory care communities.
  • Group homes.
  • Private homes.
  • Other residential care settings as appropriate.

This provider will play an important role in improving access to care for patients with chronic illness, mobility limitations, cognitive impairment, and supportive care needs.

Clinical Focus:
This role includes a strong emphasis on:
  • Mobile primary care.
  • Chronic disease management.
  • Palliative care support and symptom management.
  • Pain evaluation and treatment.
  • Dementia care and cognitive support.
  • Medication management and reconciliation.
  • Goals-of-care discussions.
  • Coordination with families, caregivers, and facility staff.

The provider will work closely with an interdisciplinary team to deliver thoughtful, patient-centered care that improves quality of life and helps patients remain safe and comfortable in their current living environment.

Required Licensure / Certifications:
  • Active Arizona license as a:
    • Physician Assistant, or
    • Nurse Practitioner.
  • Board certification preferred.
  • Current BLS certification.
  • DEA license.
