    Schedule: Day Shift | 8-Hour Days | 40 Hours/Week

    Position Overview

    Trinity Healthcare Talent Services is seeking a Travel Physical Therapy Assistant (PTA) for a Home Health setting in Sacramento, CA.

    This role involves providing in-home therapy services, assisting patients in improving mobility, independence, and functional outcomes in their home environment.

    Facility Information

    • Setting: Home Health / Post-Acute Care
    • Coverage Area: Sacramento region
    • Charting System: HomeCare HomeBase (HCHB)
    • Contract Type: LOA Coverage (Urgent Need)

    Clinical Responsibilities

    Patient Care

    • Provide therapy services under PT supervision
    • Assist with:
      • Strength and mobility training
      • Gait and balance improvement
      • Functional independence
    • Work with patients recovering from:
      • Surgery
      • Chronic illness
      • Mobility limitations

    Home Health Duties

    • Travel to patient homes
    • Evaluate home environment for safety
    • Educate patients & caregivers on:
      • Exercises
      • Equipment use
      • Home safety

    Clinical Skills

    • Assist with:
      • ROM assessments
      • Therapeutic exercises
      • Prosthetic training
      • Functional mobility training

    Documentation

    • Maintain detailed records using HCHB system
    • Complete:
      • Progress notes
      • Clinical summaries
      • OASIS-related documentation

    Collaboration

    • Coordinate with:
      • Physicians
      • PTs
      • Nurses & home health aides
    • Participate in case conferences

    Scheduling & Work Details

    • 8-hour day shifts
    • Full-time (40 hrs/week)
    • Mileage reimbursement (GSA guidelines)
    • Timesheets must be approved by supervisors

    Required Qualifications

    Active California PTA License
    Minimum 2 years experience
    Home Health experience (REQUIRED)
    OASIS experience (REQUIRED)
    BLS Certification
    Valid driver's license + auto insurance
    NPI Number

    Preferred Qualifications

    HomeCare HomeBase (HCHB) experience
    Strong communication & documentation skills
