Full-Time-Mon-Fri-8:15am-5:00 PM

• What You'll Do:
• Provide skilled physical therapy services in patients' homes under the supervision of a Physical Therapist
• Monitor patient progress and document care accurately
• Educate patients and families on treatment plans, safety, and equipment use
• Collaborate with physicians, therapists, and care teams to improve patient outcomes

Qualifications:

• Current PTA license in the state of employment
• Minimum 1 year of acute care experience required
• Home health experience preferred
• Valid driver's license and auto insurance
• Strong communication and clinical skills
