Job description:
Position Summary:
Provides physical therapy treatments and services to home care clients under the direct supervision of a qualified Physical Therapist in accordance with the Plan of Care.
Clinical Responsibilities:
- Provides services and education/training to clients in their homes following the established Plan of Care and the specific functions identified by the supervising physical therapist.
- Notifies the supervising physical therapist of any change in the patient’s condition not within the planned progress or treatment goals, and any change in the patient’s general condition and documents in clinical record.
- Documents all services provided, observations, client progress, and recommendations according to established plan of care and sign the patient record.
- Participates and documents client case conference as requested to provide information to other members of the team.
- Submits documentation to the agency as per guidelines.
- Utilizes physical means such as exercise, massage, heat, water, light, and electricity under the supervision of the physical therapist as per physicians
- Participates in the discharge planning process in coordination with the supervising physical therapist.
- Serves as a member of the home care team.
- Physical Therapy Assistant shall not:
- Perform measurement data collection or care prior to the evaluation of the patient by the Physical Therapist
- Document patient evaluation and reevaluation
- Write a discharge summary
- Establish or change a Plan of Care
- Be the sole physical therapy representative in any meeting with other healthcare professionals where the patient’s plan of care is assessed or may be modified.
Qualifications:
- Successful completions of physical therapy assistant program approved by the American Physical Therapy Association of council on Medical Education and Hospital of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
- Current and unencumbered license to practice physical therapy in the state of California
- Minimum of one (1) year experience as a Physical Therapy Assistant preferred.
- Current CPR certification
- Demonstrate good written and verbal communication skills and interpersonal skills
- Must be familiar with general use and functions of the computer such as usernames and password concepts; internet; e-mail; navigation of computer desktop
- Licensed driver with automobile insured in accordance with state requirements
- Required professional liability insurance
- Must be able to adapt to the patient’s needs and schedule changes
