Key Responsibilities:- Perform in-home PT evaluations and create personalized care plans
- Provide skilled PT interventions to enhance strength, balance, mobility, and safety
- Educate clients and families on exercises and home safety
- Collaborate with a multidisciplinary care team
- Document care using mobile-friendly EMR
Requirements:- Active MA PT license
- DPT or MPT from an accredited program
- CPR certified
- 1+ year of experience preferred (new grads welcome)
- Reliable vehicle & valid driver’s license
