A growing home health organization is seeking a dedicated Physical Therapist (PT) to join its Palm Beach County team. This is a full-time, pay-per-visit opportunity offering strong earning potential, flexibility, and the ability to make a direct impact in patients' homes and daily lives.

The ideal candidate is compassionate, independent, and comfortable managing a mobile patient caseload throughout the community.

Responsibilities

• Evaluate and treat patients in their homes in accordance with physician orders and individualized care plans

• Develop and implement physical therapy treatment plans focused on improving mobility, strength, balance, and overall function

• Educate patients and family members on exercises, safety, and ongoing care needs

• Complete timely and accurate clinical documentation

• Collaborate with physicians, nurses, case managers, and interdisciplinary care teams

• Monitor patient progress and adjust treatment plans as needed

• Maintain compliance with Medicare, state, and agency guidelines

Requirements

• Active Florida Physical Therapist (PT) license required

• Home health experience preferred, but not required

• Strong clinical and communication skills

• Ability to work independently and manage a field-based schedule

• Reliable transportation and valid driver's license required

• Experience with KanTime EMR is a plus
