Position Summary & Responsibilities

• Evaluate patients and establish PT treatment plans

• Provide mobility, gait, transfer, and strengthening interventions

• Monitor patient progress and adjust care plans

• Collaborate with interdisciplinary care team

• Complete accurate and timely documentation

Requirements

• Certification in Physical Therapy

• Active California PT license (required)

• SNF or rehab experience preferred

• Strong clinical and communication skills
