Physical Therapist - St. Charles Hospital
Job Summary:
The Physical Therapist completes initial assessments, ongoing assessments and provides skilled therapeutic interventions to patients through the use of their educational knowledge, skill, and ability. This may involve outpatients, inpatients, pediatrics and off-site locations. Services the patients as a part of the entire continuum of professional care.
Essential Functions:
Evaluates patients, gathering pertinent information from various sources, and develops an appropriate treatment plan with specific goals. Responsible for ongoing assessment, efficiency of treatment plan and revisions of plan as needed.\u202f\u202f
Provides direct therapy to patients daily following patient's plan of care and current standards of practice for Physical Therapy . Provides instruction in therapeutic procedures to patients and family.\u202f
Maintains accurate and concise records on treatment provided and patient's progress. Documents all care provided, coordination of services, orders, goals, etc. in the patient's electronic medical record in an accurate and timely manner. Provides timely co-signature for supervised physical therapy assistants, if applicable\u202f
Maintains set level of productivity without jeopardizing quality.\u202f Delivers efficient and effective care to patients meeting and exceeding department standards.\u202f
Maintains departmental policies and procedures regarding safety, infection control, time off requests and timecard recording. This will include all necessary education and yearly compliance requirements.\u202f
Supervises the physical therapist assistant's treatment interventions and professional growth.\u202f\u202f

Education:

Required Licensing & Certifications:
Licensed Physical Therapist in the state of current practice or meet all qualifications for traineeship or temporary license in the state of current practice (required)\u202f\u202f\u202f
BLS Basic Life Support (preferred upon hire, required within 30 days for BSMH, required at hire for RSFH)\u202f

Demonstrates appropriate application of age specific standards, policies and procedures and guidelines in caring for pediatric, adolescent, adult, and geriatric patients\u202f\u202f
Demonstrates general computer skills including data entry, word processing, email, and records management.\u202f\u202f
Analytical abilities to evaluate patient condition, capabilities and progress.\u202f\u202f\u202f
Interpersonal skills necessary to effectively interact with co-workers, physicians, patients and families.\u202f\u202f\u202f
Requires the ability to work well with all levels of patients, the public and other health care professionals.\u202f\u202f\u202f\u202f
