Job Details

A full-time Physical Therapist opportunity is available within a highly respected rehabilitation program serving both a Skilled Nursing Rehab setting and a Senior Living Community.

This role offers the opportunity to work alongside an experienced rehab director and collaborative therapy team while treating a diverse geriatric population focused on restoring function, independence, and quality of life.

Responsibilities:

Evaluate and develop individualized treatment plans
Provide physical therapy services in Skilled Nursing and Senior Living settings
Collaborate with PTs, OTs, SLPs, nursing staff, and physicians
Maintain timely and accurate electronic documentation
Participate in interdisciplinary care planning
Educate patients, families, and caregivers
Support functional mobility, balance, strengthening, and fall prevention programs
Contribute to positive patient outcomes and quality initiatives

Qualifications:

Licensed Physical Therapist or license eligible
Experience in Skilled Nursing, Senior Living, Rehabilitation, Post-Acute Care, or Geriatrics preferred
Strong communication and documentation skills
Experience with electronic medical records
Proficiency with Microsoft Outlook, Word, and Excel
Team-oriented approach to patient care
