About Us

Piedmont Post Acute is an 88-bed skilled nursing facility located in Piedmont, South Carolina-just 20 minutes from Greenville. Our smaller size creates a quiet, close-knit, family-oriented environment where we truly care about our residents and each other. We are seeking a compassionate and skilled Physical Therapist (PT) to join our rehab team on a PRN basis to help residents reach their highest level of independence.

Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and overall function. You'll play a key role in helping residents regain independence and improve quality of life.

Key Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, and balance
• Monitor progress and adjust plans as needed
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary team members
• Educate residents and families on treatment plans and goals

Qualifications

• Active, unencumbered Physical Therapist (PT) license in South Carolina (required)
• Degree in Physical Therapy (Bachelor's, Master's, or Doctorate)
• Experience in long-term care or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong communication and clinical skills
• Passion for improving resident outcomes
