    Responsibilities
    • Evaluate patients and develop individualized treatment plans
    • Provide skilled physical therapy interventions to patients recovering from illness, surgery, or injury
    • Work with patients who have medically complex needs in a post-acute/SNF environment
    • Collaborate closely with PTAs, OTs, SLPs, nursing staff, and families to ensure comprehensive care
    • Document progress accurately and timely to support quality care and compliance

    Qualifications
    • Licensed Physical Therapist (PT) in the state of [Insert State] or ability to obtain license
    • Experience in post-acute or SNF setting preferred, but new grads are encouraged to apply
    • Strong communication, collaboration, and clinical skills
