Physical Therapist (PT) 

 

A healthcare organization is seeking a dedicated Physical Therapist (PT) to provide rehabilitative care and help patients improve mobility, function, and overall quality of life. This role is ideal for a compassionate professional who is committed to delivering patient-centered care in a collaborative healthcare environment.

 

Responsibilities

  • Evaluate patients and develop individualized treatment plans based on clinical findings and rehabilitation goals

  • Provide therapeutic interventions to improve mobility, strength, balance, and functional independence

  • Monitor patient progress and modify treatment plans as needed

  • Establish realistic and achievable rehabilitation goals

  • Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely

  • Educate patients and family members regarding treatment plans, exercises, and recovery strategies

  • Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care

  • Supervise and support Physical Therapist Assistants as appropriate

  • Maintain compliance with professional standards, regulatory requirements, and patient confidentiality guidelines

  • Promote a safe, positive, and patient-focused care environment

  • Perform additional duties as assigned

 

Qualifications

  • Graduate of an accredited Physical Therapy program

  • Current Physical Therapist license or eligibility for licensure in the state of practice

  • Strong clinical assessment and treatment planning skills

  • Experience with rehabilitation documentation and patient progress tracking

  • Excellent communication and interpersonal skills

  • Ability to work effectively as part of a multidisciplinary team

  • Experience in rehabilitation, long-term care, skilled nursing, or geriatric settings preferred
