A healthcare organization is seeking a dedicated Physical Therapist (PT) to provide rehabilitative care and help patients improve mobility, function, and overall quality of life. This role is ideal for a compassionate professional who is committed to delivering patient-centered care in a collaborative healthcare environment.
Evaluate patients and develop individualized treatment plans based on clinical findings and rehabilitation goals
Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
Monitor patient progress and modify treatment plans as needed
Establish realistic and achievable rehabilitation goals
Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely
Educate patients and family members regarding treatment plans, exercises, and recovery strategies
Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care
Supervise and support Physical Therapist Assistants as appropriate
Maintain compliance with professional standards, regulatory requirements, and patient confidentiality guidelines
Promote a safe, positive, and patient-focused care environment
Perform additional duties as assigned
Graduate of an accredited Physical Therapy program
Current Physical Therapist license or eligibility for licensure in the state of practice
Strong clinical assessment and treatment planning skills
Experience with rehabilitation documentation and patient progress tracking
Excellent communication and interpersonal skills
Ability to work effectively as part of a multidisciplinary team
Experience in rehabilitation, long-term care, skilled nursing, or geriatric settings preferred
