Inabia is seeking a compassionate and motivated Physical Therapist to provide rehabilitation therapy services to residents within a senior living and skilled nursing community. The therapist will help residents maintain or regain mobility, independence, and overall physical health.
Key Responsibilities
Conduct comprehensive physical therapy evaluations to determine patient mobility limitations and rehabilitation needs.
Develop individualized treatment plans to improve strength, flexibility, balance, and coordination.
Provide therapeutic interventions including mobility training, strength exercises, manual therapy, and balance rehabilitation.
Assist residents recovering from surgery, illness, stroke, or chronic conditions.
Monitor therapy progress and update treatment programs based on patient performance and goals.
Educate residents and caregivers on safe mobility practices, fall prevention, and home exercise programs.
Maintain accurate therapy documentation including evaluations, treatment notes, and discharge summaries.
Work collaboratively with interdisciplinary care teams including nursing staff, physicians, occupational therapists, and speech therapists.
Participate in care planning meetings and support safe discharge planning.
Requirements
Active Physical Therapist License
Degree from accredited Physical Therapy program
Experience in rehabilitation or senior living environment preferred
