Physical Therapist (PT) - Acute Care (Per Diem) Responsibilities:
• Conduct thorough patient assessments to identify areas of need and develop individualized treatment plans • Develop and implement evidence-based treatment programs to promote patient mobility, strength, and function • Collaborate with multidisciplinary teams to ensure comprehensive care and optimal patient outcomes • Educate patients and families on proper exercise techniques, self-care strategies, and home safety modifications • Document patient progress and communicate with healthcare providers to ensure seamless care coordination

• Masters degree in Physical Therapy (MPT) or Doctor of Physical Therapy (DPT) degree • Current licensure as a Physical Therapist in the state of Connecticut • 1-2 years of experience in acute care or related field • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment with multiple priorities and deadlines
