Position Summary The Physical Therapist (PT) will provide skilled home health services to patients, focusing on improving functional independence, safety, and overall quality of life. Key Responsibilities
Perform comprehensive patient assessments and evaluations
Develop and implement individualized therapy treatment plans
Provide occupational therapy services in the patient’s home

Ensure compliance with all clinical and regulatory standards
Qualifications
Active Physical Therapist (PT) license in the state of Colorado
Reliable transportation and valid auto insurance
