Job Summary
The Physical Therapist (PT) provides direct patient care, including evaluations and recertifications, while developing and overseeing individualized treatment plans. This role includes collaboration with the interdisciplinary team and supervision of Physical Therapy Assistants (PTAs) to ensure optimal patient outcomes.

Responsibilities

  • Perform patient evaluations and recertifications to establish and update plans of care
  • Administer prescribed physical therapy treatments to relieve pain, restore function, and promote healing
  • Provide skilled therapy services in accordance with physician orders
  • Collaborate with and provide direction to Physical Therapy Assistants (PTAs)
  • Implement and modify treatment plans based on patient progress and clinical judgment
  • Monitor patient response to treatment and adjust interventions as appropriate in collaboration with the interdisciplinary team
  • Provide education and support to patients, families, and caregivers
  • Ensure accurate and timely documentation in compliance with company policies and regulatory standards
  • Attend required meetings as designated by the Director of Rehabilitation

Qualifications

  • Doctorate or Master’s degree in Physical Therapy from an accredited program
  • Current Texas licensure as a Physical Therapist
  • SNF/long-term care experience preferred
  • Strong knowledge of current treatment practices
  • Familiarity with Net Health electronic documentation system preferred
