Physical Therapist (PT) – Facility & Community-Based | Old Bridge, NJ | PRN & Part-Time | $65–$85 Per Visit
Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients at an established facility and in community-based settings throughout the Old Bridge, NJ area.
Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability.
This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule. You'll enjoy the variety of treating patients at a single established facility and in the surrounding community while maintaining the flexibility and autonomy that home health offers.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population at our established facility and in patients' homes throughout the Old Bridge area.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one treatments based on each patient's functional needs and goals.
- Treat patients at an established facility and in community-based settings.
- Manage individualized treatment plans with clinical autonomy.
- Help patients improve mobility, strength, balance, safety, and overall function.
- Complete, accurate, and timely clinical documentation.
- Communicate with patients, families, physicians, facility staff, and members of the clinical team.
- Coordinate with our administrative team regarding scheduling, patient care, and logistics.
- Build and manage your caseload based on your availability and patient needs.
Compensation:
Part-Time W-2:
- $65 per follow-up visit.
- $75 per 4-unit evaluation.
PRN:
- $72 per 4-unit follow-up visit.
- $85 per evaluation.

Qualifications:
- An active Physical Therapist (PT) license in New Jersey is required.
- Reliable transportation required.
- Strong clinical, communication, and time-management skills.
- Ability to work independently in both facility and community-based settings.
- Home health or facility-based experience is preferred but not required.
