Physical Therapist

Position Overview

We are seeking a licensed Physical Therapist to provide high-quality rehabilitation services to patients throughout the Morris County region. The ideal candidate will be experienced in patient evaluations, treatment planning, therapeutic interventions, patient education, and clinical documentation while working independently in a home health environment.

Key Responsibilities

Perform comprehensive patient evaluations and functional assessments

Develop individualized treatment plans and rehabilitation programs

Implement therapeutic interventions to improve mobility, strength, and functional independence

Educate patients and caregivers regarding treatment goals, home exercise programs, and safety measures

Document evaluations, treatments, progress notes, and discharge summaries accurately and timely

Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams

Maintain compliance with clinical, regulatory, and documentation standards

Support quality improvement and patient outcome initiatives

Required Qualifications

Active New Jersey Physical Therapist License

Graduate of an Accredited Physical Therapy Program

Ability to work independently with strong clinical judgment

Excellent communication and patient care skills
