The physical therapist works under the supervision of leadership. Evaluates, plans, and implements treatment programs for the rehabilitation of patient, as directed by a physician, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation.

Essential Technical/Motor Skills
- Ability to communicate clearly to patients, fine motor skills required.
Interpersonal Skills
- Displays a friendly, positive attitude and communicates with patients, families, physicians and co-workers in a professional manner. Ability to communicate both verbally and written.
Essential Physical Requirements
- Must move, kneel, crouch, stoop, reach, stand, push, pull, and lift. Must follow good body mechanics.
Essential Mental Abilities
- Assess, evaluate, treat, and educate patients. Must be adaptable to the many challenges of differing home environments. Also includes explaining, forecasting, organizational skills and time management.
Essential Sensory Requirements
- Visual, hearing and feeling skills.
Exposure to Hazards
- Frequent exposure to communicable diseases, toxic substances, body fluids, road and weather hazards.
Other Skills and Abilities
- Must be registered and successfully pass background check with the Family Care Safety Registry and other applicable state registries. Must be a licensed driver with an automobile that is insured in accordance with the state and/or organization requirements and is in good working order.
- Provides physical therapy services to patients according to a written physician's plan of care.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patient in the use of prosthetic device.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Prepares and submits clinical and progress summaries based on attainment of goals.
- Participates in discharge planning
- Provides a physical therapy consultation to home families/caregivers when indicated.
- Participates in Interdisciplinary Group to define and update the comprehensive plan of care.
- Supervises Physical Therapy Assistants according to organization policy and state regulations
- When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two weeks.

Education
- Graduate of accredited physical therapy program - Required
Work Experience
- 1 Year - Appropriate experience as a physical therapist - Required
- 1 Year - Community/home health experience - Preferred
Licenses and Certifications
- Licensed Physical Therapist in State of Missouri - Required within 60 Days
- Basic Life Support (BLS) - Required within 90 Days
- Driver''s License - Valid In State - Required Upon Hire
Travel Requirements
- Must be able to travel between various system facilities and off-site locations as needed. - Required
- Provides physical therapy services to patients according to a written physician's plan of care.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
