This position includes a 5000 sign-on bonus. Schedule: Part Time to Full Time hours - 32-40 hours. Pay period: Monday to Friday - 8 a.m. to 4:30 p.m.

Job Responsibilities
- Deliver Direct Client Care:
• Conduct comprehensive evaluations
• Develop personalized treatment plans with short-term and long-term goals
- Maintain Accurate Client Records:
• Document services accurately and timely using Vivies EMR system
• Ensure compliance with all payer sources and authorization requirements
- Facilitate Follow-Up Programs:
• Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
- Collaborate and Communicate Effectively:
• Work closely with clients, families, physicians, and the interdisciplinary team to provide care
- Ensure Regulatory Compliance:
• Complete and document supervisory visits with PTAs and RACNAs in accordance with state and regulatory guidelines

Requirements
- Current Physical Therapist license in the state of Minnesota
- Strong collaboration and teamwork skills
- Clear and compassionate communication abilities
- Proficient in English for effective communication with staff, clients, and visitors
- Successful completion of a state-mandated background check
- Able to physically perform the duties required by the position
- Must obtain CPR certification within 6 months of hire
