This position includes a 5000 sign-on bonus. Schedule: Part Time to Full Time hours - 32-40 hours. Pay period: Monday to Friday - 8 a.m. to 4:30 p.m.

Job Responsibilities

  • Deliver Direct Client Care: • Conduct comprehensive evaluations • Develop personalized treatment plans with short-term and long-term goals
  • Maintain Accurate Client Records: • Document services accurately and timely using Vivies EMR system • Ensure compliance with all payer sources and authorization requirements
  • Facilitate Follow-Up Programs: • Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
  • Collaborate and Communicate Effectively: • Work closely with clients, families, physicians, and the interdisciplinary team to provide care
  • Ensure Regulatory Compliance: • Complete and document supervisory visits with PTAs and RACNAs in accordance with state and regulatory guidelines
  • Other Duties as Assigned: • The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive

Requirements

  • Current Physical Therapist license in the state of Minnesota
  • Strong collaboration and teamwork skills
  • Clear and compassionate communication abilities
  • Proficient in English for effective communication with staff, clients, and visitors
  • Successful completion of a state-mandated background check
  • Able to physically perform the duties required by the position
  • Must obtain CPR certification within 6 months of hire
