Job Description:
We are looking for a dedicated Physical Therapist to join our dynamic home health team. This role involves working closely with a team-based unit to manage and deliver comprehensive patient care plans in residential settings. The ideal candidate will be instrumental in providing exceptional care and will have the opportunity to work in a supportive environment that values professional growth and work-life balance.
Primary Duties and Responsibilities:
- Provide physical therapy services in patients' homes within the Jackson area and its surroundings.
- Collaborate with healthcare teams to ensure the delivery of effective patient care plans.
- Utilize state-of-the-art technology to streamline charting processes and enhance care delivery.
- Support patient and family education programs to promote health and wellness.
- Manage time effectively to meet critical deadlines for patient documentation.
- Maintain compliance with all local and state licensing requirements for physical therapy.
Qualifications:
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited institution.
- Valid Physical Therapist license in the state of operation.
- Minimum of one year of healthcare experience preferred.
- Current driver’s license with the ability to travel locally to patient homes.
- Proficient in the use of tablets, mobile phones, and EMR software.
- Ability to self-manage schedules and critical charting deadlines.
