Job Description:
We are seeking a compassionate and dedicated Physical Therapist to expand our client’s exceptional home health care team. Positioned in a team-based environment, this role focuses on the effective management and delivery of patient care plans.
Primary Duties and Responsibilities:
- Assess, plan, and implement physical therapy interventions for patients in a home health setting.
- Work closely with a multidisciplinary team to tailor care plans to patient needs, ensuring optimal outcomes.
- Utilize cutting-edge technology to efficiently manage documentation, allowing more time for patient interaction.
- Participate in a robust onboarding program to thoroughly understand company protocols and patient care standards.
- Mentor and support from clinical educators and supervisors to foster professional growth and expertise in home health care.
- Regularly travel locally to provide direct care in patients' homes, managing your schedule to meet care delivery needs.
Qualifications:
- Bachelor’s or Master’s degree in Physical Therapy from an accredited college or university.
- Valid Physical Therapist license in the state of Michigan.
- Minimum one year of healthcare experience, with a preference for candidates with home healthcare experience.
- Valid driver's license and reliable transportation for local travel.
- Strong organizational skills with the ability to meet critical deadlines in a dynamic environment.
- Proficiency in using electronic medical records (EMR) and mobile devices for clinical and communication purposes.
