Responsibilities:

The Physical Therapist works directly with individuals to restore or improve physical abilities, promote behavioral changes, adapt surroundings, and/or teach new skills, to achieve optimal physical function and prevent disabilities, in accordance with principles and practices of the profession. The Physical Therapist may provide evaluation and treatment of patients in acute care, nursing home, short-term rehabilitation and/or outpatient settings. The Physical Therapist oversees services provided by Physical Therapy Assistants and aides. They are required to work as part of a multi-disciplinary care team. They may assist in quality improvement and new programming initiative development. This position reports to the Rehab Supervisor, Rehab Manager and Director of Rehabilitation Services.

  • Maintains licensure by obtaining required CEUs as determined by NYS Practice Act
  • Adheres to professional standards, policies and procedures, federal, state and local requirements and Joint Commission (TJC) standards.
  • Performs a comprehensive physical, psychosocial, and safety assessment as evidenced by clinical documentation. Identifies those functional problems that require physical therapy intervention based on an accurate assessment of the patients status.
  • Must be capable of understanding and carrying out prescribed therapy services as requested by physicians and/or midlevel providers.
  • Develops a plan of care to promote optimum function and safety, as documented in the clinical record.
  • Completes all required documentation following evaluation, treatment and procedures and ensures that documentation reflects care provided and outcomes achieved.
  • Demonstrates the ability to evaluate and treat the assigned number of patients per day, and determine the need for follow up visits to maximize the functional outcomes
  • Maintains current knowledge and awareness of insurance regulations related to provision of therapy services
  • Acts fiscally responsible in use & request of supplies, and in submitting patient charges in a timely manner.
  • Ability to handle and operate therapy equipment, including but not limited to: safe patient handling equipment, exercise machines, and/or modalities
  • Coordinates patient care with other clinical team members ensuring optimal patient care and communication
  • Maintains and enhances personal and professional knowledge, as well as promotes professional standards in physical therapy
  • Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.
  • Promotes effective working relations and works effectively as part of a team inter and intra departmentally to facilitate the department’s ability to meet its goals and objectives.
  • Additional duties as assigned.
Qualifications:

Education:

  • 4 Year/Bachelor's Degree in Physical Therapy from an accredited College or University, required
  • Master's Degree of DPT, preferred

Experience:

  • Experience as a Physical Therapist prior to employment, preferred
    • New grads will be considered

License/ Certification:

  • NYS license to practice PT, or a valid temporary permit as per NYSED licensing requirements, required
  • BLS CPR - within 6 months of date of hire, required

Physical Requirements:

  • Light Work - Exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg control requires exertion of forces greater than that of sedentary work and if the worker sits most of the time, the job is considered light work.
  • Occasional balancing, climbing, crouching, repetitive motion, stooping
  • Frequent typing/clerical/dexterity, grasping, kneeling, lifting, pulling, pushing, reaching, seeing/monitor/computer use, standing, talking, visual acuity (color, depth, perception), walking

Founded in 1956, Cobleskill Regional Hospital is a 40-bed, not-for-profit hospital accredited by The Joint Commission. Cobleskill Regional Hospital provides inpatient care for general medical conditions, an inpatient short-term rehabilitation unit, and more than 20 outpatient specialty services in areas such as heart and cancer care, women’s health, orthopedics, lung health, general surgery, urology, diabetes care and treatment, hearing, and neurology -- in addition to physical therapy and rehabilitation services. Cobleskill Regional Hospital’s Emergency Department is staffed at all times by highly qualified physicians and nurses who provide rapid assessment and expert treatment 24 hours a day, 7 days a week. Cobleskill Regional Hospital is the only hospital in Schoharie County. 

Cobleskill Regional Hospital has made keeping up with changing medical technology a top priority. In 2014 - 2015, major enhancements were made to the Hospital's Emergency Department which resulted in an expanded number of treatment rooms, Medical Imaging technologies continue to be a primary focus with the addition of a low-dose, 64-slice CT scanner made possible in part by a federal appropriation secured by Congressman Paul Tonko and awarded through a U.S. Health Resources and Services Administration HCOF grant.
